• This membership fee recurs monthly unless cancelled prior to end of term.
• No refunds or exchanges unless proof of medical exceptions.
• Must be used within 13 months of purchase activation. 
• Fee Charged unless proof is provided of illness
• Fee Charged unless proof is provided of move 50+ outside of zip code where the office is located

Service Terms
• All included services rendered in-office.
• Appointment required.
• Services must be used in the month (or another period) in which they are earned.
• Additional units will be available at the time of service for a discounted rate.
• We reserve the right to refuse membership to anyone at our discretion.
• We reserve the right to change the terms of this membership at any time without notice.
