Cancellation Terms
• 30 days notice to cancel after 6 months agreement.
• Services are not transferrable.
• Unused monthly service value can be credited toward future aesthetic services at Ball Dentistry as long as membership is active.
• We reserve the right to edit the terms of this membership.
• Opus Plasma benefit cannot be used in the first six months of membership period.

Service Terms
• All included services rendered in-office.
• Appointment required.
• Services must be used in the month (or another period) in which they are earned or will be forfeited.
• Additional units will be available at the time of service for a discounted rate.
• We reserve the right to refuse membership to anyone at our discretion.
• We reserve the right to change the terms of this membership at any time without notice.
